When to use Print PDF By Mail Complete Form Online, Sign & Mail Applications 403(b) Custodial Account Booklet Use to open a new 403(b) account. The booklet also includes Appointment of Financial Professional and 403(b) Transfer request forms. This booklet should not be used to open any other type of account. Regular Account Application Use this form to open a new Individual, Joint, UGMA, Trust, Corporation and Association/Partnership accounts. This form is also used to open an account for SOME self-directed retirement plans. This form should NOT be used for ETFs or to open any type of IRA. Rydex Retirement Kit Booklet w/ IRA Application Use to open IRA, rollover IRA, ROTH and SEP accounts. The kit also includes Appointment of Financial Professional and IRA Transfer/Direct Rollover request forms. This kit should not be used to open any non-IRA account. Forms Check Signature Card Submit one signature card for each account number to establish check writing privileges against the Rydex U.S. Government Money Market Fund. All registered owners must sign in order to receive checks. Not available for A or C class share or retirement accounts. 403(b) Distribution Form Use to request all distributions from 403(b) accounts. The included Supplemental Employer Information form must accompany all requests. IRA Distribution Form Use to request distributions from IRA accounts. Use the Required Minimum Distribution form for required minimum distributions. Deposit Slips May be used to accompany a deposit. 403(b) Supplemental Employer Information Form Use this form to provide information and employer approval for all 403(b) transactions (excluding exchanges within your Rydex account) Account Change Form (No Signature Guarantee) Use to change your address, dividend reinvestment options or beneficiary. Also use to appoint an authorized individual (non-financial professional). Account Change Form (Signature Guarantee) Use to change or add wiring instructions and change account registration. Retirement Account Transfer/Direct Rollover Request Use to transfer IRA assets from another IRA custodian. Mutual Fund Transfer Form Use to transfer account assets from another firm (non-IRA accounts ONLY). On Demand and Automatic Investment Request Form Use to provide ACH instructions for future use or to set up an automatic investment on a schedule you select. Required Minimum Distribution Form Use to request required minimum distributions from IRA accounts. All other IRA distributions should use the IRA distribution form. Systematic Withdrawal Form Use to set up systematic withdrawals by check or via ACH (electronic transfer to your bank). 403(b) Custodial Account Asset Transfer Election Form Use to transfer an existing 403(b) custodial account from your current custodian to Rydex. Appointment of Financial Professional Use to appoint or change a financial professional. Transfer On Death Form Use to add transfer on death instructions to an account (non-IRA accounts ONLY). W-9 Form (Request for taxpayer ID) IRS form W-9 is required after opening an account online or upon request.
